Common questions

Who qualifies for NJ DDD Support Coordination?

Adults 21+ who are determined eligible by the NJ Division of Developmental Disabilities and enrolled in the Supports Program or Community Care Program. Medicaid enrollment is required — if you're not there yet, we'll help you navigate the steps.

Does Support Coordination cost my family anything?

No — Support Coordination is funded through the DDD program for eligible individuals. It does not come out of pocket and does not reduce the services in your loved one's budget.

Can we switch to Priority Group Services from another agency?

Yes. Families can change Support Coordination agencies. We'll walk you through the transfer so services continue without interruption.
