Priority Group Services NJ provides Community-Based Supports for adults enrolled in the New Jersey DDD Supports Program, in Bergen, Essex, Hudson, Monmouth and Ocean counties. A trained Direct Support Professional (DSP) works with your loved one by the hour, at home and in the community, on the goals in their plan.

From your plan to the first visit
Talk to your support coordinator
Ask them to add Community-Based Supports to the plan and to list Priority Group Services NJ as the provider. You can also send us a referral and we will contact the coordinator with your permission.
We meet you
We learn the person's interests, routines, language and the times that work, then match a DSP.
Support begins
Your DSP works on the plan's goals, and we keep the support coordinator updated on progress.
Who is eligible: adults 21 and over enrolled in the DDD Supports Program with Community-Based Supports in their plan, living in Bergen, Essex, Hudson, Monmouth and Ocean counties. If Priority Group Services NJ is already your support coordination agency, DDD's conflict-free rules mean your coordinator will help you choose another provider for this service.

What are Community-Based Supports?
Community-Based Supports is a DDD Supports Program service. A Direct Support Professional (DSP) works one-to-one with the person, by the hour, at home and out in the community, on the goals in their plan.
How many hours can we get?
Hours come from your loved one's Individualized Service Plan and DDD budget. Your support coordinator adds the service to the plan; we then build a schedule within the hours authorized.
Do families pay for these hours?
Authorized hours are paid through your loved one's DDD program. We will tell you up front if anything you ask for falls outside the plan.
We are in the Community Care Program. Can you help?
In the Community Care Program, hourly one-to-one help is called Individual Supports. Call us and talk to your support coordinator about which services fit your loved one's plan.
Can we meet the DSP before they start?
Yes. We introduce the DSP before the first shift, and if the match is not right, tell us and we will look again.
Our support coordinator works for Priority Group Services NJ. Can you still be our provider?
No. DDD's conflict-free rules mean the agency that coordinates your services does not also provide them. Your coordinator will help you choose another approved provider.
