Refer someone for support coordination (Atlantic, Burlington, Camden, Gloucester and Somerset counties) or for DDD services such as hourly DSP support and respite (Bergen, Essex, Hudson, Monmouth and Ocean counties), and we will call back within two business hours. Who can make a referral?
Anyone: the person themselves, a parent or guardian, a school or transition coordinator, a hospital or clinic, or a support coordination agency.
What happens after I send it?
A member of our New Jersey team calls back within two business hours on business days. We confirm what is needed, explain the next DDD step, and, with the family's permission, contact the support coordinator.
What information should I not put in the form?
Please leave out diagnoses, Medicaid numbers and Social Security numbers. We collect anything we need privately on the phone.
