In the landscape of Medicaid services in New Jersey, the waiver application programs of the Community Care Program (CCP) and the Supports Program stand as pivotal gateways. Community Care Program (CCP)

The Community Care Program (CCP) stands as a crucial aspect of our commitment to Medicaid beneficiaries in New Jersey. CCP allows individuals to access an array of approved waiver services and supports, empowering them to reside in their communities and avoid institutionalization.

For individuals with intellectual and developmental disabilities, CCP serves as a lifeline. Without this program, Medicaid funding could only extend to those residing in institutions. Waivers like CCP enable states to assist individuals with specific service needs in avoiding institutionalization and fostering community inclusion.

Enrollment in CCP is available to individuals meeting Division service eligibility criteria and the specified clinical level of care for Intermediate Care Facility of Individuals with Intellectual Disabilities (ICF/IID). Additionally, specific Medicaid requirements regarding income and resources apply.

CCP caters to adult individuals aged 21 and above, offering support across various living arrangements—be it with their families, in licensed residential settings, or in unlicensed settings. Our program is dedicated to providing choice among approved waiver services and providers. Through the guidance of Support Coordinators, individuals and their families can craft Individualized Service Plans, ensuring access to tailored services aligning with their unique needs.

Please note, individuals enrolled in CCP cannot simultaneously participate in other Home & Community Based Services (HCBS) or Managed Long Term Services & Supports (MLTSS) programs, including the Supports Program.

Supports Program

The Supports Program, an integral part of the Comprehensive Medicaid Waiver (CMW), earned approval from the Centers for Medicare & Medicaid Services (CMS). As part of a statewide Medicaid service reform, this initiative prioritizes community-based services and enables New Jersey to access increased federal funds.

Designed for adult individuals aged 21 and above, the Supports Program caters to those living with their families or in unlicensed settings. Its primary goal is to enhance support services for adults with developmental disabilities while significantly reducing the backlog of individuals awaiting support and services.

Enrolled participants in the Supports Program gain access to employment/day services and individual/family support services tailored to their assessed level of need. Flexibility is key here—individuals and families can choose from a range of support options, guided by Support Coordinators who assist in devising Individualized Service Plans and connecting them with appropriate services.

With the exception of individuals enrolled in other Home & Community Based Setting (HCBS) or Managed Long Term Services & Supports (MLTSS) programs, all eligible adult individuals under Division services and Medicaid can access the Supports Program.
