Community Based Supports in New Jersey

Community Based Supports (CBS) is one of the most-used services in New Jersey’s DDD Supports Program — one-on-one support that helps adults with intellectual and developmental disabilities build real independence in their own homes and communities. Priority Group Services provides CBS across Essex County and northern New Jersey with support professionals matched to each individual’s goals.

What Are Community Based Supports?

Community Based Supports provide direct, individualized assistance with daily living — delivered where life actually happens: at home, in the neighborhood, and out in the community. A trained Direct Support Professional (DSP) works one-on-one with your loved one on the skills and routines identified in their person-centered plan.

The goal is never to do things for someone — it is to build the skills, confidence, and connections that let each person do more for themselves, at their own pace.

Cooking, budgeting, personal care routines, and household management — practiced hands-on.

Community participation

Support getting out: errands, appointments, recreation, and local programs.

Social & communication skills

Building relationships and confidence in everyday interactions.

Travel training

Learning to navigate transportation safely and as independently as possible.

Medication reminders, appointment tracking, and safe-at-home habits.

Goal-based support

Every hour maps to the outcomes in the individual's NJ DDD service plan.

How It Works With Your NJ DDD Plan

Services are delivered through New Jersey’s Division of Developmental Disabilities (DDD) programs. Your Support Coordinator adds Community Based Support to the individual’s person-centered plan, and we take it from there — matching staff, building the schedule, and reporting progress against the plan’s goals. If you don’t yet have a Support Coordinator, we can point you in the right direction.

Who can receive Community Based Supports in NJ?

Adults 21+ who are eligible for the NJ Division of Developmental Disabilities (DDD) Supports Program or Community Care Program can typically access CBS through their individualized budget. Your Support Coordinator adds it to the service plan.

Where does the support take place?

Wherever it is needed — the individual’s home, family home, or out in the community. CBS is intentionally flexible so support follows real life, not the other way around.

Can we choose or change the support professional?

Yes. We match DSPs carefully to each individual and family, and if a match is not working we will re-match — consistency and trust come first.
