Kiyana Grayer, MBA, MHA, is a mission-driven healthcare and human services executive with over a decade of leadership experience across developmental disabilities, Medicaid-funded programs, compliance, residential services, housing initiatives, and workforce strategy. She has successfully led highly regulated organizations, partnering with boards, agencies, and community stakeholders to ensure quality, sustainability, and person-centered outcomes. Kiyana also maintains hands-on support coordination expertise, specializing in ISP development, NJCAT assessments, and service monitoring for individuals with intellectual and developmental disabilities. Known for building strong teams and compliance systems, she values continuous learning, reading, and is a proud mother of two daughters with uniquely mirrored birthdays.

Chizoba Alice Osuagwu is the Manager, supporting the DDD program at Priority Group Services. She manages service coordination, referral intake, and compliance tracking, focusing on delivering high-quality solutions through collaboration, strategic thinking, and attention to detail. Alice is committed to advancing the DDD team’s goals while maintaining a consistent and professional presence across all platforms. She brings a passion for continuous learning and professional growth, driving results and supporting the team’s success.

Ellen Mae Libaton is the Program Director's Executive Assistant. As a strategic partner to the Program Director, She coordinates the operational and administrative functions of the NJ DDD Priority Groups project to ensure the seamless delivery of disability services. She serves as a primary liaison between state stakeholders and service providers, managing high-priority deliverables and navigating complex regulatory landscapes to support the program’s mission of empowering New Jersey’s developmental disability community.

Rochelle works with the New Jersey DDD - Priority Group Services as a Senior Program Coordinator. She oversees onboarding and recruitment for DDD staff, including DSPs, Support Coordinators, and PT staff. Her responsibilities include conducting background checks, ensuring completion of all required documentation, assigning mandatory CDS training modules, and following up with staff as needed. She also handles communication with internal and external partners and conducts outreach to generate referrals and new leads.
